Following are the items to bring for Admission

A. Documents

  • For admission registration, you should bring original copies of the following documents:
  • Identification Proof of the patient. (Pan Card, Driving License, Voters card, Aadhaar Card)
  • Passport (Non-Resident Indian)
  • All earlier Medical Reports and Consultation papers.
  • We would like you to register yourself with Lifeline Hospital at time of your first visit.
  • UHID will be issued to you after registration. We request you to preserve bring the UHID Registration slip while your visit to Hospital.
  • At the time of Admission, the security deposit is taken which gets adjusted against the final bill or refunded after settlement of claim.

B. Money and Valuables

Please do not carry excess cash or valuables while coming for hospitalization.

  • Instruction for Corporate/ Company Patient's
  • If you are a employee of Company / Organization which is empanelled with Hospital, you are requested to submit an Authority Letter from your Company/ Organization for Registration and Admission.
  • Medicines/ Consumables will not be provided on credit basis until the Authority letter from Company/ Corporate is submitted to the Admission Counter of Hospital.
  • If you wish to opt a higher-class accommodation than authorized, you need to pay security deposit as per deposit schedule applicable to respective class of accommodation category. Make a note that all charges will be proportionately increased as per class of accommodation. The same will be settled at the time of discharge/ final settlement of bill.
